Vibration White Finger Explained
Vibration white finger (VWF) is caused by damage to the small arteries and arterioles in the fingers resulting from prolonged, regular use of vibrating hand-held power tools. The vibration causes structural changes to the vessel walls, making them prone to vasospasm — sudden contraction that cuts off blood flow. When this happens, the affected fingers turn white (blanch), becoming cold and numb. On rewarming, blood flows back causing the fingers to turn blue and then red, often accompanied by considerable throbbing pain.
Attacks are typically triggered by exposure to cold — cold weather, cold water, or handling cold materials or equipment. Frequency, duration and severity of attacks all worsen as the condition progresses without reduction of vibration exposure.
The Stockholm Vascular Classification
VWF severity is staged using the Stockholm Workshop vascular classification:
- Stage 0V: No attacks
- Stage 1V: Mild — occasional attacks affecting only tips of one or more fingers
- Stage 2V: Moderate — occasional attacks affecting distal and middle phalanges of one or more fingers
- Stage 3V: Severe — frequent attacks affecting all phalanges of most fingers
- Stage 4V: Very severe — frequent attacks in Stage 3V with trophic changes in the fingertips

Establishing Liability for VWF
The Control of Vibration at Work Regulations 2005 (and the older guidance under the NIOSH/BS standards applicable in earlier periods) set out the employer's duty to manage vibration risks. In older claims from the 1970s and 1980s, the courts assess what the employer knew or ought to have known about vibration risks at the relevant time, applying guidance from the Stockholm Workshop paper (1987) and the British Standard BS 6842 (1987) as a benchmark.
